LAST NAME, FIRST NAME.(Try to be creative with this, please don't just use the name from another anime. You could find a translation site, and you don't necessarily need to love your name, it could easily be something to laugh at later. Do what makes you happy, that's the main thing.) HAILING COUNTRY, VILLAGE.(This is self explanatory. The country your character comes from followed by the village in that country.)

[/b] [ 12 - 16 is a good age for Genin level shinobi, much older than that and you would be considered unskilled or lazy. That may be what you're going for though, right?]
GENDER: Pick the gender of your character. Male, female, possibly even both, or a genderbender.
STATUS: You got your plain hetero and homosexuals. What about a technosexual, pansexual, or possibly even someone turned on by a sunny day. If the shoe fits, wear it.
RANK: You must start at the genin rank, unless permission is given to you. If you got permission to be a higher rank provide the person who gave the option to you, and have them vouch for it.
CLAN: You don't need to be from a clan. Everyone will want to be in a clan, most likely. Remember, you are only limited by your own creativity.
BLOODLINE: God, this is the worst section. Everyone wants a demon or bloodline, if you can stray from this I applaud you. Not every character needs sage mode, forbidden jutsu, a demon, or a sharingan. Trying to get by without it will actually make you a much better Role-player and it will increase your creativity in combat.

THE HEIGHT, & WEIGHT.THE EYE COLOR, & HAIR.(Maybe even go into further detail. I'd like to know that poetic side of you. It's much more impressive to supply 100 words for hair than it is to use this small section as your crutch.) [/b] Here you can describe your skin, hair, eyes, mouth, toes, fingers, muscle tone, bone structure, facial hair, nose, ears, etc. This list is never ending, and it should be the easiest thing to accomplish. You just need to describe the picture that you're using. Every picture is worth a thousand words, damn it! Milk it for all its worth.
I understand at some point you may feel that it's too much work for a simple profile, or maybe it seems too wordy; however, this will make your description skills much better in the rp as well. Instead of seeing a “mountain” you might be able to add some pizazz to it; “The earth reached for the sky like a woman's nipple in an blizzard, tearing through the clouds, opening what might be Olympus to all the mortals that climb it.” My description is simple, but you get the idea.
DISTINGUISHING FEATURES: Do you have a scar/tattoo? Are you missing an Arm? In fact you could put anything here. Glasses, weird eye color, painted nails on a male, a second head, five arms, etc. Use your imagination, it's your only limitation to describing your character.
CLOTHES: Describe the clothes on your back. Simple enough, describe what people will see when you walk around on a normal day. Maybe even supply several outfits, such as what one might wear in summer, fall, or winter. This is about the same difficulty as describing your physical appearance. In fact, I would like to see about the same amount here as you had in your physical appearance, because most people cover about 80% of their bodies with clothes.

DISLIKES: Your likes and dislikes set you apart from every other ninja. They are what can turn an ordinary ninja into a quirky one. Your character could like hobos or dislike wood. These can affect your ninja til his/her dying breath, and can always make things more fun in the rp.
PERSONALITY: This whole section, including likes and dislikes is rather important. HOWEVER, it doesn't need to be as thorough as your appearance. Lets face it, you will rp your personality. This is not as dire as your appearance, but it will give people an idea about what to expect from you, which will allow you to form more compatible teams and choose the people that will be more fun to rp with. The more fun the personality, the more fun the rp. Just make sure that your character is fun for you, because that's all that matters.

NATURE RELEASES.ADVANCED RELEASES.(The advanced releases are only for people in clans that have abilities with them. However, the regular nature releases allow you versatility in combat. Pick elements that compliment each other, maybe even pick elements that are polar opposites to make your character capable of doing more with his jutsu, or you can do what I do: pick elements that sound cool then end up not using them because they aren't needed.) [li] If you have any items put them here, as well as the amount to keep from god modding in a battle. You shouldn't have infinite Kunai, though you may carry a hell of a lot.

[/li][li] Why do you need weapons. Sure, they are a nice asset, but remember you can always think about these things before deciding. You could use a fricking pot as a makeshift mallet! Use your brain and you shall not be disappointed.

SUMMONS: You don't need to have a summon, it's actually rather rare. If you do have one, pick an animal you'll have fun rping. Maybe take a whack at having a bunny summon?

TEAM MATES: This will be acquired through the rp, Naruto is highly group oriented. The better a team can work together the more epic you can be in the rp. A single force can easily be taken out with the right team, which is also why even the evil junkies have team mates to assist them.
HISTORY:Maybe you were delivered by a Stork, this can be why your character acts the way he was. TRY TO STAY AWAY FROM CLICHÉ. Too many fucking people put that there parents died, or their family was massacred. Please stay away from this, it's boring and unoriginal. You don't need a normal life, but please stay away from cliché as much as possible. Some people don't like hundreds of Sasukes running around.
